It should always be closed unless you have the heat on, otherwise in the middle of summer you would always be heating your car unless you have the AC on.
You can get rid of it if you wish but you will not be happy with the results when it gets hot out.
you will also need a pressure plate and clutch.
and I think you will need a slave cylinder but I'm not 100% sure cause I have never done this.

From the spec's of the ram you must be running on a pretty old server.
For me it would depend on time and money.
If time and money isn't really a problem. Do a rebuild, do it right which will make it last a lot longer.
If you need the car up and going and don't have the money replace it. But don't just find one and drop it in you still need to replace seals...
I remember something on Myth Busters here lately where they tested this and I was thinking that having the AC on used more gas then having the windows open and they did there testing at around 50 mph if I remember right.
If you have the title you can go to Toyota and get a new key. Really isn't that much. A buddy of mine lost his keys and had a new one in a week or so for less then 30 bucks I think.
If you had the stock head bolts and never re torqued them it could have happened. I have heard a few people say they have worked lose before. That added push from the exhaust could have pushed it over the edge.
Seems pretty odd but stranger things have happened.
