I would rebuild the whole engine but disagree with the felpro comment. For a stock rebuild a felpro headgasket is just as good as any other brand. A modified turbo should use a MHG as insurance.

Any details on what happened? Have you done anything to it lately, did anyone else drive it or work on it? It looks and sounds like bad timing to me but you haven't given us any background.
You put it in the right spot but installed it wrong. The cylinder slides through the slot and acts as a pivot. Take a closer look and it will make sense.
