UPDATE: My head is junk... warped and the cams have been eating away at everything around them... off by about 12 thousands of an inch Ordered a head from http://cylinder-heads.com/ cost was 425 shipped, and free box and shipping label to send core back. It is a 7mge head...

what color wires?

Air Conditioning fan relay connector... you are missing the relay, see the hole right above it, that is for the bolt. I know the connector is circle, not sure aboute the wire colors. I could be wrong though... just a guess

really? I have about the same temperatures as you and never got this, running german castrol. What type of oil do you run? Do you know why this happens?

Milkshake plus lots of white smoke plus loss of coolant plus overheating = BHG Do a compression to be postitive, but i'd bet it is a BHG.

if you find a fix for this please post it, I've had the same issue (idle only at about 2400 though) and it was not my throttle, and not my TPS, i'm replacing the IAC now to see if that does it

Dropped the head off at the machine shop, it was 115.77mm no warp from the last resurface. Used http://enginesbyadvanced.com/ Getting the head resurfaced, cleaned, cams washed, inspected. Cost 416 they've done supra heads before, kind of expensive, but this shop does mustangs that run 6...

Greddy wins out then... its 217 and 1.5mm I'll have the machine shop let me know exactly how big the head is and compare that to original spec. If 1.5 is almost exactly what I need (I think it will be) I'll order that. (Anyone want to buy a 1.2mm HKS?) :)
