Most of the time it's the gasket...the nuts can loosen up (especially at the #6 & # 5 runners) and cause the leak. It's pretty common for the head exhaust studs to strip...the torque spec is only 29 ft/lbs for that reason. Doesn't do a lot for keeping the manifold clamped down. IMO, it's money well spent to helicoil the exhaust stud holes when doing a rebuild....you can take the torque up to 40 ft/lbs without stripping.
The newer version exhaust manifolds have a reinforcing ridge down each runner and are heavier as well...the cracking between runners that was common on the earlier version doesn't really happen anymore.
If you get a new exhaust gasket, use Toyota. They revised the gasket to a multi-layer steel...much better than the old style. In fact, the turbo gasket available now looks like the one the NA always used
