That being said, bend/flex/wiggle your CPS wires. Try to reposition them as much as possible. Don't be a tard and go pull on them like you're trying to start a lawnmower though! :biglaugh: We're just trying to locate one or more wires that may have a short/open in it. Use zipties if necessary to bundle them 'snuggly' (is that a word?) together or to hold them in a different position.
There may be a vibration at that particular rpm that causes your CPS wires to vibrate in just the right...or wrong way, causing your problem.

A bad O2 signal will not cause the ECU to enter fail safe or back-up mode.
When you checked for IC piping leaks, did you pressurize the piping and IC up to the TB?
I would also take a closer look at the CPS wiring.
