How can it have no vacuum upon start if it's connected directly to the intake manifold?

In the factory configuration the FPU VSV is connected to the vacuum manifold with its source being the large port. When active the three way VSV prevents vacuum from reaching the FPR while at the same time venting its diaphragm to atmosphere thus increasing fuel pressure. That happens for about two minutes max and only then under certain conditions.

The PS idle up is also connected to the vacuum manifold. Control of that function is done by a hydraulically activated VSV mounted on the PS pump.

^Correct! Just wanted to get you back

OP- The large port is for FPU VSV, PS idle up and the BOV. You can sorta trace it in that link. The two nipples on the USDM TB are for EGR and EVAP.
