put the closed end of a wrench on the firewall side and get a very very long extension on a properly sized 1/4 drive socket wrench and you should be good.

Keep trying. you're not going to get it right away. The car should be jacked up so you can get around underneath it. Find out what works best for you.

get several long extensions, go from above the back of the transmission and thread your extensions all the way to the bolt. you'll need a swivel adapter on the end too.

there's a good write up somewhere, i'll try to find later.
found it:

6) Locate upper starter bolt with your hands...you can't see it unless your bellhousing is invisible :). Place your 14mm box or combination wrench on the nut side this time. Use a 14mm 3/8" socket, universal (swivel), 16"-20" extension and ratchet to get to the bolt. Starting about where the clutch slave cylinder is, you'll have to guide the socket/swivel over the bellhousing to the bolt. Loosen and remove upper starter bolt. At this point, the starter should be loose and moveable.

I'm quite familiar with this. I've changed this at least 3 times. It takes me 10 mins to take out and 15-20 to put in now lol. I take both of the bolts out from under the car. With the top bolt, i just use a ratchet with a 14mm socket. I reach over the top of the starter, feel around and mate the socket with the bolt. Make sure that you put your hand up there with the handle of the ratchet first. Then loosen until i can do it by hand. Putting another one in is the fun part. Any questions, e-mail me at dcrusupra@yahoo.com.

Thats exactly how I do it. I've had the tranny out like 3 times and thats how I get that bolt/nut out.
