More clutch problems.....

Okay, I finally got it torn apart. My throw out bearing was busted. So I get it back together last night and it drove fine, a little hard to go into gear but it was still fun. Well I park it and go to drive it to go wash it and it grinded. I pumped the clutch pedal and I heard something let go. All over again the same stuff happened as before (won't go into gear while the car is on and sounded like the fork was grinding against the flywheel). So I think I busted the throw out again. Do you think I am putting it together wrong (throw out assembly)?

i sheared off a bolt head and bent the remaining bolt on my fork pivot point.Not sure if its to do with the uprated pressureplate or it may have been loose/not square so its getting pushed on a angle.

So I have it all torn apart AGAIN. The snap ring came off the back side. So should I just order all new parts for the clutch or what (snap rings etc.). Also, I checked tsrm for how to put it together and I am not 100% sure I got all of the insides together (snap ring, thrust cone, etc.) but it worked fine. If anybody has any pics of how they put it together than I would appreciate it. Thanks!!
